Guest Speaker: Debbie Neil. Barwon Health. "Advanced Care Planning.
Advanced Care Planning is a process for making and writing down future health care wishes in advance and only comes into effect if and when you become unwell and are unable to communicate your wishes for yourself. It is a consultative process and involves a fair bit of thinking, talking with those who are concerned and often reflection about complex and sensitive issues. Advanced Care planning ensures your future medical treatment is in your hands. It also assists in the removal of guilt for careers.
Advanced Care Planning can include:
- Appointing a Medical Enduring Power of Attorney. This person or agent is only empowered to make medical treatment decisions on your behalf only if you are not able to make or communicate them yourself. (Other Powers of Attorney, such as a General or a Financial Power of Attorney are not authorized to make medical decisions.)
- Completion of a Refusal of Treatment Certificate. This document records your instructions for limiting the treatment of your current illness.
- Writing down of other wishes for future medical care. This is a Statement of care document that enables you to outline your health care values, and is a good opportunity to discuss these with those who would be involved in your future care.
Copies of the documents should be given to the relevant people e.g. your Medical Power of Attorney, family or friends, your doctor and or the Hospital.
It is possible to change or cancel these documents in the future if there is a change in your requirements or circumstances.
Information about Advanced Care Planning is available in many clinics in Geelong and district and help is available if required.
Towards the end of life approximately 80% of people are in receipt of care so what you want is worth thinking about and telling someone about it!
Membership dues are overdue!
Please note it is essential that apologies be recorded by 12noon on Mondays. Also the Club has to pay for members who have not apologized or not turned up!
Phone number is 0457 315 900. Thank You.
